Lipid Class Composition in the Freshwater Pearl Mussel Margaritifera margaritifera from Rivers of the White and Barents Sea Basins.

The contents of total lipids (TL) and individual lipid classes were studied in the mantle, foot, and hepatopancreas of the freshwater pearl mussel Margaritifera margaritifera from eight northwestern Russian rivers belonging to the White and Barents Sea basins. Tissue specificity of the lipid composition was observed in the studied tissues; i.e., the hepatopancreas had several times higher contents of TL and dominant lipid classes. A predominance of triacylglycerols among energetic lipids in the hepatopancreas was a distinctive feature of pearl mussels from the Lotta, Sredniy Verman, Mashjoki, and Nautsiyoki rivers, whereas cholesterol esters predominated in mussels from the rivers of the Pyaozero water system, which is further south. Furthermore, the cholesterol/phospholipid ratio (an indicator of the structural and functional state of membranes and a marker of temperature adaptation) was the highest in pearl mussels from the Lotta, Sredniy Verman, Mashjoki, and Nautsiyoki rivers. This may be related to corresponding adaptive changes associated with preparation for the winter period, given a more northern location of the rivers. The results of lipid status analysis in pearl mussels from different habitats may be useful for monitoring studies aimed at preserving and restoring populations of the species in the wild.

Borrelia miyamotoi meningoradiculitis complicating ocrelizumab treatment for multiple sclerosis: A report of three cases.

Ocrelizumab is an anti-CD20 monoclonal antibody that is highly effective in multiple sclerosis (MS) but is associated with an increased risk of opportunistic infections that may be difficult to diagnose. We report three MS patients treated with ocrelizumab who developed severe meningoradiculitis. Routine investigations failed to identify any pathogen, whereas metatranscriptomic analysis of cerebrospinal fluid (CSF) detected Borrelia miyamotoi RNA. All patients improved after appropriate antibiotic therapy. B. miyamotoi should be considered in anti-CD20-treated MS patients presenting with meningoradiculitis, and CSF metatranscriptomics should be used to investigate undiagnosed central or peripheral nervous system infections, particularly in immunocompromised individuals. Ocrelizumab is a highly effective treatment widely used in MS but has been associated with an increased risk of infection. We report three cases of B. miyamotoi infections in patients receiving ocrelizumab in which routine laboratory tests failed to detect the pathogen.

Duloxetine repositioning: Investigation of antifungal and antibiofilm activity against fluconazole-sensitive and resistant Candida Spp. strains.

The increase in fungal infections, the limited therapeutic arsenal, and the emergence of resistance pose a global health problem. Candida spp. stand out as opportunistic pathogens that cause superficial and invasive diseases. Thus, the search for new therapeutic alternatives, such as drug repositioning, is necessary. Duloxetine (DUL), a serotonin-norepinephrine reuptake inhibitor antidepressant used to treat depression, has demonstrated antifungal activity and potentiates the effects of conventional antifungals in vitro. The objective of this study is to evaluate the in vitro activity of duloxetine (DUL) against resistant Candida spp. strains. To this end, the minimum inhibitory concentration (MIC) of DUL alone and in combination with conventional antifungals was determined in order to evaluate the type of interaction between them, as well as the minimum fungicidal concentration (MFC). The activity of DUL against mature and developing biofilms was evaluated, in addition to the possible antifungal mechanism of action. DUL showed MICs of 16 to 128 µg/mL with a fungicidal action profile, and when combined there was synergistic interaction with amphotericin B (AMB). In biofilms, DUL exhibited antibiofilm activity and enhanced AMB activity against developing, but not mature, biofilms. Mechanism of action analysis showed that DUL promoted oxidative stress, evidenced by increased reactive oxygen species (ROS) production, reduced GSH levels, mitochondrial depolarization, and phosphatidylserine externalization, suggesting the activation of the apoptotic pathway as a possible mechanism of cell death. The pro-oxidant effects of DUL were evident in C. albicans strains deficient in antioxidant defenses (cap1Δ and gpx3Δ) compared to the wild-type strain. DUL exhibited antibiofilm activity and enhanced AMB activity against developing, but not mature, biofilms.

Discovery of sugarnitriles A and B, and structural reassignment of SF-2140, from a sugarcane endophytic actinomycete Amycolatopsis sp. JS-O27.

Three rare cyano-substituted indole N-glycosides, SF-2140 (1) and its two previously undescribed derivatives, sugarnitriles A (2) and B (3), were isolated from a sugarcane endophytic actinomycete Amycolatopsis sp. JS-O27. The planar structures of 1-3 were established by HRESIMS data and detailed 1D/2D NMR spectroscopic analysis, and the conformations of the sugar moieties were deduced by the analysis of 3 J H-H coupling constants and NOESY data, and comparison of their electronic circular dichroism (ECD) spectra. Compounds 1 and 3 exhibited moderate to weak antibacterial activity against Escherichia coli at 100 μg/mL.

Clinical and virological characteristics of hepatitis B virus genotype E infection: a case series.

Hepatitis B virus (HBV) genotype E, mainly found in sub-Saharan Africa, exhibits low genetic diversity, unique molecular markers and high recombination potential but remains under-studied. This study describes the clinical and virological characteristics, mutational profiles, antiviral treatment responses and phylogenetic data of patients with confirmed HBV genotype E infection in a tertiary-care centre in northern Italy. A retrospective monocentric study was conducted at ASST Spedali Civili di Brescia, Italy, from 2015 to 2023. Patients with documented HBV genotype E who underwent genotypic resistance testing (GRT) for clinical reasons were included. Direct sequencing of the S/POL region was used for genotyping and mutation analysis, interpreted via Geno2pheno [hbv] 2.0 and Stanford HBV resistance tools. Eight patients were identified, mostly male (62.5%) and of African origin (87.5%), with a median age of 38.5 years. GRT was performed in six (75%) patients because of persistent viremia despite antiviral treatment, five (83.3%) of whom were HIV/HBV coinfected with detectable HIV viral load. Lamivudine resistance mutations (L180M, M204V) were found in one (12.5%) case, tenofovir resistance-associated mutations (M267L/I) in two (25%) and immune escape mutations in three (37.5%), including T126I, D144E and G145R. Despite GRT, treatment was not modified in four (50%) patients due to poor adherence concerns. Therapy adjustments led to viral suppression in three (37.5%) cases. In our setting, HBV genotype E infections largely occurred among migrants from endemic regions. Although nucleos(t)ide analogues are effective across genotypes, suboptimal adherence may hinder viral suppression and promote resistance.

From talk to action: A randomized controlled design on the role of huddles in shaping safety climate and reducing missed nursing care.

Missed nursing care compromises patient safety and care quality worldwide. Although inadequate staffing, high workloads, and poor work environments contribute, brief structured team interventions may reduce care omissions. Proactive huddles aim to enhance communication, coordination, and early risk identification, yet the mediating role of safety climate remains unclear. To evaluate the effectiveness of proactive huddles in reducing missed nursing care and examine the mediating role of safety climate. Randomized controlled design. Data were collected between July 2022 and May 2023 from ten hospital wards, pair-matched by type and size and randomly assigned to proactive huddles or usual practice. The sample comprised 180 nurses (85 intervention, 95 control). The MISSCARE Survey, Safety Organizing Scale, and NASA Task Load Index were administered pre- and post-intervention across three shifts. Linear mixed-effects models and moderated mediation analysis were conducted. Proactive huddles were associated with improved safety climate compared with usual practice (b = -0.217, p < .001) and with reduced missed nursing care (b = 0.123, p < .001). Higher safety climate was associated with lower missed nursing care (b = -0.184, p < .001). Moderated mediation indicated a greater mediated reduction in missed nursing care via safety climate in the intervention group (difference in average causal mediation effect = -0.038; 95% CI [-0.059, -0.018]; p < .0001). Proactive huddles may offer a feasible, time-efficient strategy for reducing missed nursing care. Safety climate may be one mechanism linking structured communication with improved care quality, alongside other team-level mechanisms.
