Venetoclax-Induced Symmetrical Drug-Related Intertriginous and Flexural Exanthema (SDRIFE): Implications for Diagnosis and Treatment Continuation.

Symmetrical drug-related intertriginous and flexural exanthema (SDRIFE) is an uncommon type of cutaneous drug eruption, most frequently associated with antibiotics such as amoxicillin. Reports of SDRIFE induced by anticancer agents remain limited. We describe a case of SDRIFE in a 77-year-old man treated with venetoclax for acute myeloid leukemia. The eruption developed shortly after drug initiation and resolved rapidly following drug withdrawal without systemic involvement. Notably, venetoclax was continued for a certain period after onset without progression of symptoms. This case highlights the importance of recognizing SDRIFE in patients receiving chemotherapy and suggests that continuation of the causative agent may be feasible in selected cases.

Direct immersion single drop microextraction with hydrophobic deep eutectic solvent for the isolation of β-lactams and tetracyclines from milk.

The hydrophobic deep eutectic solvent (HDES) consisting of thymol and coumarin was used as an extraction medium during the isolation of the veterinary antibiotics from group tetracyclines (lymecycline LYM, oxytetracycline OTC, tetracycline TC) and β-lactams (amoxicillin AMS, ampicillin AMP). The use of deep eutectic solvents in miniaturized liquid-liquid extraction ensures the elaboration of a procedure consistent with the rules of green analytical chemistry. Single drop microextraction (SDME) technique enabling a radical reduction of HDES volume was developed for isolation of tetracyclines and β-lactams antibiotics from the bovine milk samples. The obtained microextracts were analyzed followed by the liquid chromatography method connected with tandem mass spectrometry (LC-MS/MS). The linear concentration range (1.10-8 mol L-1-7.10-5 mol L-1) of the studied compounds for elaborated HDES-SDME-LC-MS/MS method enables their determination in milk samples according to the normalized values (EU Regulation 37/2010).

Intracranial abscess and otitis interna treated by temporal burr-hole craniotomy and ventral bulla osteotomy in a Maine coon cat.

A 1-year-old Maine coon cat was presented with acute onset of central vestibular syndrome and recurrent generalized tonic-clonic seizures. Computed tomographic imaging of the head revealed a left-sided intracranial lesion consistent with an abscess, associated with bilateral otitis media/interna, with more severe involvement of the left tympanic bulla. Based on clinical and imaging findings, the cat underwent surgical intervention consisting of a left temporal burr-hole craniotomy for abscess drainage combined with a unilateral ventral bulla osteotomy. Purulent material was collected during surgery and submitted for bacterial culture, which yielded Staphylococcus aureus sensitive to amoxicillin-clavulanic acid and metronidazole. Postoperative management included intravenous antimicrobial therapy followed by oral treatment after discharge. Transient vestibular dysfunction and Horner's syndrome were observed after surgery and resolved completely within 3 mo. At a long-term follow-up, the cat was neurologically normal. This case highlights the successful surgical management of an otogenic intracranial abscess in a cat and supports unilateral surgical treatment guided by clinical and imaging findings. Key clinical message: Intracranial abscesses secondary to otogenic infection should be considered in cats that have central vestibular clinical signs and seizures. A combined surgical approach using burr-hole craniotomy and ventral bulla osteotomy, guided by imaging findings, can result in favorable clinical outcomes.

N4-Acetyl-Sulfamethoxazole Bladder Stones Caused by Trimethoprim-Sulfamethoxazole Prophylaxis: A Report of Two Cases.

The combination antimicrobial trimethoprim-sulfamethoxazole (TMP-SMX) has a broad spectrum of activity and is used as a prophylactic for various infections. Drug-induced urolithiasis is a rare side effect of TMP-SMX that results from the precipitation of its metabolite N4-acetyl-sulfamethoxazole. There are only two reports of bladder stones associated with TMP-SMX, both of which occurred in patients with a history of recurrent urinary tract infections (UTIs). At our institution, an 80-year-old male with a complex medical history including benign prostatic hyperplasia (BPH) and a chronic prosthetic joint infection on long-term amoxicillin and TMP-SMX presented for evaluation of an incidental 2.4 cm bladder stone. Surgical intervention was initially deferred due to patient preference, but he later developed gross hematuria and decided to undergo cystoscopy with cystolitholapaxy and bilateral retrograde pyelogram. The procedure was completed without complications. Analysis of stone fragments revealed that they were 100% N4-acetyl-sulfamethoxazole. Months later, a 70-year-old male with multiple sclerosis and neurogenic bladder complicated by recurrent UTIs managed with a suprapubic catheter and TMP-SMX prophylaxis was found to have multiple bladder stones causing obstructive symptoms. These stones were similarly treated with cystolitholapaxy and identified as 100% N4-acetyl-sulfamethoxazole. Bladder stones caused by TMP-SMX have been rarely reported in people like our 70-year-old patient with an abnormal urinary metabolic profile, indicated by a history of urolithiasis or UTIs. Our other patient did not have either of these risk factors, but he did have lower urinary tract symptoms due to BPH. His case may serve as a caution to the use of TMP-SMX in the setting of chronic urinary stasis due to bladder outlet obstruction.

Penicillin-resistant Streptococcus dysgalactiae subsp. equisimilis and the association with penicillin-binding protein mutations: a population genotypic and phenotypic study.

Streptococcus dysgalactiae subspecies equisimilis (SDSE) is increasingly recognized as a cause of human disease. Despite decades of penicillin use, SDSE is near universally penicillin-susceptible. Here we report and characterize by whole genome sequencing (WGS) three penicillin-resistant SDSE isolates (minimum inhibitory concentration [MIC] ≥0.256 mg/L) isolated from blood and skin in an individual with recurrent cellulitis on penicillin prophylaxis. The isolates carried 11 penicillin-binding protein (PBP) PBP2x, five PBP1a, and one PBP2b mutations that were distinct from the only previously reported WGS-characterized penicillin-resistant SDSE isolates (n = 4). We then took a genotype-led approach to test the MICs of penicillin and amoxicillin for 63 isolates with PBP2x, PBP1a, and PBP2b variants selected from a well-characterized database of invasive SDSE isolates from Australia (2011-2023, n = 436). PBPs were highly conserved in Australian invasive SDSE genomes. Only a single isolate carrying a PBP2x variant with five mutations around the penicillin-binding cleft found in 1/436 invasive isolates demonstrated a four-fold rise in penicillin MIC, although remaining below clinical breakpoints. Strains with PBP2b and PBP1a variants alone were not associated with raised MICs. To provide broader context for PBP variants, we then examined a global database of SDSE genomes (n = 1,160). PBP2x mutations in isolates with raised and/or resistant penicillin MICs were found in only nine global SDSE database genomes. SDSE penicillin resistance is rare and associated with multiple PBP mutations, particularly PBP2x. WGS with phenotypic testing of novel PBP variants could be used to monitor emerging mutations associated with raised penicillin MICs.

Prevalence, antimicrobial resistance and detection of resistance genes of Escherichia coli and Salmonella spp. isolated from captive European wild boars reared in Brazil.

The expansion of invasive European wild boar (Sus scrofa scrofa) populations in Brazil and the increasing commercialization of this species raise concerns about their role as reservoirs of zoonotic and antimicrobial-resistant bacteria. This study aimed to determine the prevalence, antimicrobial resistance profiles, and detection of virulence and resistance genes in Escherichia coli and Salmonella spp. isolated from captive wild boars. A total of 100 rectal swab samples were collected from animals raised on commercial farms in Goiás State, Brazil. One Salmonella isolate (1%) was detected and identified as serotype O:6,8, showing phenotypic resistance to ampicillin, amoxicillin, and sulfonamides, but no virulence or resistance genes were detected. E. coli was isolated from 56% of samples, with 98.2% of isolates classified as multidrug-resistant and all presenting antimicrobial resistance index values above 0.2. High resistance rates were observed for sulfonamides (96.8%), tetracycline (92.1%), amoxicillin (87.3%), ampicillin (85.7%), doxycycline (81.0%), whereas most isolates remained susceptible to ceftriaxone. Virulence genes were detected in a subset of isolates, including tsh (29.8%), papC (12.3%), and iss (7.0%), with some isolates presenting gene combinations associated with increased pathogenic potential. The eae gene was not detected, and no β-hemolytic activity was observed. These findings indicate a low occurrence of Salmonella but a high prevalence of multidrug-resistant and potentially pathogenic E. coli in captive wild boars, highlighting their potential role as reservoirs of antimicrobial resistance within a One Health context.
