Measles in Ugandan Children Under 18 Months of Age: A Retrospective Study of Case-based Surveillance, 2018-2024.

Measles remains a significant public health threat in Uganda, with regular outbreaks. The current national immunization schedule offers the first measles-containing vaccine dose (MCV) at 9 months, and a second dose at 18 months. We investigated the epidemiology of measles in Ugandan children aged 18 months and younger. We analyzed the national measles surveillance data from 2018 to the first quarter (Q1) of 2024, probing the demographic and geographic distribution. We categorized children as under 6 months, 6 to under 9 months, and 9-18 months. Suspected measles cases were classified as clinical, epidemiologically linked, laboratory-confirmed or discarded (not measles). 1316/2274 (57.9%) of the suspected cases were diagnosed with measles. One hundred and forty-nine cases (11.3%; 95% CI: 9.7-13.0) were aged under 6 months, 390 (29.6%; 27.0-32.0) 6 to under 9 months, and 777 (59.0%; 56.0-62.0) 9-18 months. 717/1208 (59.4%; 57.0-62.0) of vaccine-eligible children, with a captured vaccination history, had received at least 1 dose of MCV. 59.5% (95%CI: 55.0-63.0) of measles cases were unvaccinated vaccine-eligible children. Several districts did not achieve the WHO-recommended measles surveillance threshold of at least 2 discarded cases per 100 000 population. Most measles cases among Ugandan children ≤18 months are unvaccinated. Infants nine to 18 months old comprise most of the measles cases (59.0%; 777/1316). To achieve better control of measles transmission, review of the current vaccination policies with strengthened vaccination and surveillance systems are recommended.

Examination of the Publication Quality of Abstracts of Systematic Reviews and Meta-Analyses on Measles, Published between 2009-2023 and Indexed in the PubMed Article Database.

Measles, a highly contagious, yet vaccine-preventable disease, is currently experiencing a notable resurge in numbers, in developing countries. With limited reading time, physicians often rely on structured summaries, and well-prepared abstracts can encourage them to read the full article, facilitating patient care. We aimed to examine the reporting quality of article abstracts about measles. Indirectly/directly address measles and its vaccine, scrutinizing on systematic reviews and meta-analyses published from 2009 to the present, and indexed in the open-access PubMed article database. With the widespread use of abstract checklists like PRISMA-A in reading systematic reviews and meta-analyses, the message intended to be conveyed can be adequately delivered to the reader by abstracts only, respecting standard rules and requirements for reporting. We used a scoring system for compliance with PRISMA-A checklist in reading measles-related reviews published over the last 15 years. On average, the abstracts were "very highly" compliant with the expected reporting criteria: The year of publication (with 2020 as the timepoint) did not make any difference in reporting quality, but structured abstract were significantly more likely to convey their message in an "expected" manner, based on PRISMA-A criteria. Using standard guidelines in evaluating reporting quality of different publications and emphasizing its importance for the writers and readers, alike, will be encouraging for improved presentation of original/filtered research results, with the goal of conveying valid and reliable health-related information, in a time-efficient way.

Development and evaluation of a virus like particle based vaccine candidate against Indian Isolate of porcine circovirus 2d in pig.

Porcine circovirus type 2d (PCV2d) is the predominant genotype etiological agent of porcine circovirus-associated disease (PCVAD), causes substantial economic losses to the swine industry worldwide, including in India. Since currently available vaccines are mainly based on the PCV2a genotype, which is unable to offer cross protective immunity against emerging PCV2d strains, development of PCV2d specific vaccine is necessary. In this study, an indigenous virus-like particle (VLP)-based vaccine candidate was developed using the capsid coding gene (orf2) of an Indian PCV2d isolate expressed in a baculovirus-insect cell system. Recombinant capsid protein expression and self-assembly into VLPs were confirmed by Western blotting and transmission electron microscopy. The purified VLPs showed favourable thermostability as well as pH stability and elicited strong humoral and cellular immune responses in in vivo porcine model. A delayed booster immunization strategy resulted in prolonged antibody persistence. In further, in vivo challenge study with virulent homologous PCV2d virus vaccinated animals were protected against PCV2 clinical signs. Further, absence of detectable viral DNA in swab and tissue samples, and markedly alleviated histopathological lesions compared with unvaccinated controls animals. Overall, these findings suggest that the indigenous PCV2d VLP vaccine candidate is highly immunogenic and provides protective immunity against homologous PCV2d challenge, highlighting its potential indigenous vaccine candidate for controlling PCVAD against PCV2d.

Widespread hypovitaminosis D among adolescent girls in India: The role of dietary vitamin D intake and socioeconomic status.

Vitamin D deficiency remains a major public health concern among adolescents, particularly in low- and middle-income countries. This study assessed vitamin D status and examined socioeconomic, dietary, and lifestyle factors associated with vitamin D deficiency among adolescent girls in India. A cross-sectional study was conducted among 1009 girls aged 11 to 18 years attending government schools. Serum 25-hydroxyvitamin D [25(OH)D] concentrations were measured to determine vitamin D status, and information on dietary intake, socioeconomic characteristics, anthropometric indicators, clinical symptoms, and lifestyle factors was collected using standardized procedures. Vitamin D deficiency (<20 ng/mL) was identified in 50.0% of participants, while 37.9% were classified as insufficient (20-29 ng/mL), 11.0% as sufficient (30-100 ng/mL), and 1.1% as having concentrations above 100 ng/mL. A substantial proportion of participants reported dietary intakes below recommended levels for energy, protein, fat, calcium, and vitamin D. Multivariable logistic regression analyses indicated that lower socioeconomic status and inadequate dietary vitamin D intake were independently associated with an increased likelihood of vitamin D deficiency. Other dietary, clinical, and lifestyle factors showed weaker or nonsignificant associations after adjustment for potential confounders. These findings indicate that hypovitaminosis D is highly prevalent among adolescent girls in India and highlight the important contribution of modifiable dietary and socioeconomic factors to vitamin D status. Strategies aimed at improving dietary vitamin D intake and addressing nutritional inequalities may contribute to reducing the burden of vitamin D deficiency in this vulnerable population.

Development of an oral Lactobacillus casei-based recombinant vaccine expressing GCRV-II multi-epitope fusion protein and its protective efficacy against grass carp hemorrhagic disease.

Grass carp hemorrhagic disease caused by grass carp reovirus (GCRV) results in severe economic losses in aquaculture. Vaccination is an effective strategy for controlling viral infections. The subunit vaccines are considered promising because of their safety, strong immunogenicity, and ease of production. In this study, antigenic epitopes derived from the outer capsid proteins VP4, VP5, and NS38 of GCRV genotype II (GCRV-II) were identified, and oral Lactobacillus casei recombinant vaccines were developed to evaluate their protective efficacy in grass carp. Based on sequence homology, epitope prediction and structural analyses, three dominant epitopes, VP4274-367, VP5105-336 and NS38143-338, were selected and fused into a recombinant protein. The immunogenicity of fusion protein was confirmed by enzyme-linked immunosorbent assay (ELISA). Injection of the fusion protein significantly enhanced the expression of immune-related genes, increased serum neutralizing antibody levels and reduced the expression of inflammatory cytokines. After GCRV-II challenge, the vaccinated fish showed a survival rate of 66.7%, compared with 6.7% in the injectable control group, and exhibited the lowest tissue viral loads among all groups. To develop an oral vaccine suitable for aquaculture, the fused epitope sequence was inserted into the secretory expression vector pVE5523 and expressed in L. casei. The recombinant strain showed high tolerance to heat and acidic conditions and stably colonized the intestine for up to 14 d. Oral administration of feed containing freeze-dried recombinant L. casei significantly upregulated immune-related genes (ifn1, cd4-2, mhc-Ⅱα,igm and igt) as well as lysozyme and complement C3 levels. After GCRV-II challenge, orally vaccinated fish reached a survival rate of 70%, much higher than the 23.33% survival observed in the oral control group, with significantly decreased tissue viral loads. These results demonstrate that the recombinant L. casei vaccine LC-VP4274-367-VP5105-336-NS38143-338 is a promising oral vaccine candidate for protecting grass carp against GCRV-II infection.

A Thematic Analysis Exploring Cultural Elements of Dance Movement Therapy in India.

India has a long, rich, and diverse culture in the arts, and dance and movement is one of the vital elements. This impacts the practice of dance movement therapy (DMT) in India. Research done on DMT mostly highlights the Western perspective. There is insufficient research on DMT in the Indian context. This study is an attempt to understand the cultural elements that are included by dance movement therapists (DMTh) in their DMT practice in India. This study followed a qualitative design underpinned by a constructivist paradigm. To explore the cultural influences the narratives of 24 dance movement therapists (having an experience of 3 years and more) practicing in India were taken into account. Data was collected through semi structured interviews. Using thematic analysis the interview transcripts were analysed. Analysis of interviews identified major themes such as dance being part of the therapists upbringing (emotional, social, cognitive, and physical), DMT and Mental Health (bridging the gap between training and practice and belief in the therapeutic value of dance), DMT as a therapeutic modality (creative exploration and movement improvisation, non-judgemental and safe space, less stigma, rewarding and fulfilling), cultural influences in DMT (influences from various dance forms and movements, cultural influences in the multimodal approach, influences from the clients) and DMT- a melting pot of East and the West. The findings indicate that DMThs in India draw inspiration from Indian classical dance forms, regional folk dances, Bollywood dance, and yoga. The DMThs keep in mind their multimodal approach which includes the use of props, music, and mythology are also culturally relevant. This study has implications for DMThs and mental health professionals for better understanding and incorporating cultural elements within the therapeutic space of DMT in India.
